Adult Day Health Care Center is a licensed community-based day health program that provides services to older persons and adults with chronic medical, cognitive, or mental health conditions and/or disabilities in a coordinated program of professional and compassionate services in a community-based group setting. Services are designed to provide social and health services to adults who need supervised care in a safe place outside the home during the day. They also afford caregivers respite from the demanding responsibilities of caregiving. The Program stresses partnership with the participant, the family and/or caregiver, the primary care physician, and the community in working toward maintaining personal independence.
Our Center provides care Monday through Friday during the hours of 8:30am to 3pm. These general services are offered and individualized based on an individual's assessed needs:

- Nursing care- observation, assessment, and monitoring of individual general health and medication management
- Social Services- individual and group counseling, care coordination and caregiver support
- Physical ,Occupational and Speech Therapy- rehabilitation services designed to help individuals to maintain or regain their ability to participate in the activities of daily living.
- Transportation - door-to-door service
- Meals- participants are provided hot meals and nutritional counseling, those with special dietary needs are offered special meals
- Personal care- help with toileting, grooming, eating and other personal activities of daily living.
- Therapeutic activities- exercise and interaction with other participants in planned activities appropriate for individual's conditions
Adult day center offers people with Alzheimer's and other dementias the opportunity to be social and to participate in activities in a safe environment and a much needed break for the caregiver. We know that memory loss affects every participant differently. Our team is trained to recognize and value these differences and to provide personalized care.

Chronic disease management and health monitoring is designed to provide exceptional support to individuals with chronic and complex illnesses allowing them to remain at home safely , reduce or manage symptoms and avoid unnecessary hospitalizations and nursing home placement. Our specially-trained interdisciplinary team of nurses, therapists, social workers, mental health clinicians and certified nursing aides understand the challenges faced by individuals with a chronic illness. We mobilize our specialized clinical resources to ensure each individual achieves and maintains the best possible level of independence and quality of life.

For many, home is the most comfortable place of all. Especially following a stay in the hospital or skilled nursing facility, having the love and support of family—in familiar surroundings—can enhance the healing process significantly. At Golden Castle, we enable individuals to continue living at home whether independently or with family members, while maintaining their independence as they receive skilled nursing and rehabilitation at the Center in a supportive group setting. Including adult day health care services for an individual living at home may be appropriate for a patient who has experienced an illness, hospitalization or surgery which requires recovery or healing or has a new medical problem that calls for a change in daily lifestyle.
